Okay, so I did the config with raspbpi-config and there was absolutely no difference to the config.txt.
Anyway, I got some htop stats now, and there's very little CPU usage from autoapp when idling, but when I click around it jumps up to around 25%. However, since there's practically nothing else running on the pi, and it idles at a total usage of around 10%, there should be plenty of CPU to go.
One curious thing I found: When enabling the top bar in Android Auto, performance actually goes UP a bit. It's still far from fluent, but noticeably better than when the bar is disabled.
Oh, and one more thing I've found. Performance is a lot better over USB. However, I find it hard to believe WiFi to actually be the limiting factor. I've tried connecting both using a router and directly to an hotspot on a Pixel 3XL lying less than 10cm from the Raspberry.

The Android Auto UI. Unfortunately, my RPI (3) doesn't support 5ghz wi-fi so I can't really try that.
But using 2.4ghz when connecting to headunit reloaded on my galaxy pad doesn't show the same behaviour.
It's the wi-fi on the RPI particularly bad? I get decent download speeds, but I suppose latency would be an issue. But still, even when less than a meter away, it gets incredibly laggy.
