It definitely seems like a problem with hardware, rather then the open auto software.

I figured out a configuration that works for me, maybe this will help you?
Microphone
Got an Andoer, after trying several other ones that should but didn't work with Raspbian:

Raspbian Audio Device Settings
Sound card: USB PnP Sound Device (Alsa mixer) (Default) > make sure to set as Default
Capture: Microphone > make sure to enable
config.txt
dtparam=audio=off
gpu_mem=256
dtoverlay=vc4-fkms-v3d
disable_splash=1
Android Auto Pro
Audio Settings: Speech audio channel=Off > Note that my phone has a direct Bluetooth connection with the stereo, music outpout is also bypassing the Pi, but can be controlled by it
Works like a charm for me now + the microphone is good to place around the car
