Currently we are investigating issues with external BT dongle on the newest RPI4 and Buster OS. We will let you know the results.
Nevertheless, please note that OAP for RPI 4 is still a Beta version.

Yeah, seems to be just a pi 4 issue, hfp works fine on a pi 3

I now experience this same issue as well. tried with both the built in BT and 2 different external usb ones. BT media playback works fine. Calls can be placed and received through OAP/AA. But the incoming/outgoing audio from calls doesn't work. Also using Pi4. Strangely enough, this worked for me for several weeks, but now it doesnt.
I was having this issue on my RPi4 as well and here's how I fixed it: I removed my external usb sound card, used a usb mic instead. Then just plugged the audio directly into the Pi's 3.5mm port. I did have to re-image everything because I was having trouble getting sound back after swapping but after that it all just worked.
